Form QR code for sign-ups and leads
A short form that opens from print, with every answer in one place.
A form QR code opens a hosted form on the phone: name, e-mail, a choice, a date or any field you add. Answers arrive in the app, ready to export, so a poster or a stand becomes a sign-up sheet that nobody has to type up later.
A stand without a clipboard
A visitor scans the stand sign and leaves name and e-mail in a few taps.

Questions and answers
Before you print
- Is it free?
- Fixed codes are free: make as many as you like, download a PNG with no sign-up, and get every other format and print files with a free account. Editable codes come with a plan, a pass, Keep-alive or the 14-day trial.
- Can I change the link after printing?
- Yes, with an editable code. It holds a short link you control, so you can point the printed code somewhere new any time, and the next scan opens the new page. A fixed code keeps the link it was made with.
- Can I use my own domain?
- Yes. A custom domain is included on passes, from Starter, and during the trial. Starter covers one subdomain, for example go.yourbrand.com, and apex domains start on Pro.
- What happens if I stop paying?
- Editable codes work to the end of your paid period, and after a failed payment every code keeps working for 14 days. Turn codes back on within 12 months. Fixed codes keep working as long as their links do.
How do I make a QR code for a form?
Build the form in the app and download its code, or paste the link of a form you already have into the generator above.
Where do the answers go?
Into the app, where you can read them and export them.
Can I link a Google Form?
Yes. Paste its link into the generator for a free fixed code.
Can I edit the form after printing?
Yes, with an editable code. Add or remove fields and the printed code opens the new form.
Can I ask for marketing consent?
Yes. Add a consent tick box; each answer records whether it was ticked.
Is a form QR code free?
A fixed code to your own form is free in SVG, PNG, JPG, WebP, PDF and EPS. The hosted form needs an editable code.
How many fields should a form have?
Three or four. Every extra field costs answers, so ask only what you will use.
Can people fill in the form on any phone?
Yes. The form opens in the phone's browser, fits small screens and checks each field before it is sent, so answers arrive complete.
Turn print into sign-ups
Link your form above, or build one in the app.
