Collect customer feedback with a QR code
Ask while the visit is fresh. One scan opens your form, and answers arrive before the guest has left.
To collect customer feedback with a QR code, link the code to a short form, such as Google Forms, Typeform or your own survey, and print it where the visit ends: the table, the room, the receipt or the waiting area. Paste the form's address above, keep it to three questions and test the scan.
Feedback before they leave
A card with the bill. Guests scan and answer three quick questions on their phone.
How to set up a feedback code
Make a short form
Three questions at most: a rating, what went well, what to fix. Short forms get finished on a phone.
Paste the form's link
Copy the share link of the form and paste it above. Choose editable if you may swap the form later.
Print a friendly ask
"Two minutes to tell us how we did?" next to the code says how long it takes and why it matters.
Read and reply weekly
Set a time each week to read the answers and fix one thing. Customers notice when feedback changes something.
Worked example: a dental practice
- Where the code sits
- A card at the reception desk and one in each treatment room, with "How was your visit today?" above the code.
- What it asks
- A score from 1 to 5, the waiting time in a few words and one thing the practice could do better.
- What changed
- Comments about long waits led to a text message when the dentist runs late, and the comments stopped.
Fixed or editable for feedback?
-
Fixed QR code (static)
A fixed code to one form is free and fine if the form will stay the same. If you ever replace it, every printed card has to be reprinted.
-
Editable QR code (dynamic) Our pick for this job
An editable code lets you switch to a new form or survey tool without reprinting, and its scan count shows how many people opened the form against how many finished it.
Getting more answers
Ask at the right moment
At the end of the visit: with the bill, at checkout, on the room card. Not at the door on the way in.
Keep it anonymous
Make the name field optional. People say more when they do not have to sign their answer.
Close the loop
A small sign saying "You asked, we changed it" tells people their answers are read and brings more of them.
Questions and answers
Before you print
- Is it free?
- Fixed codes are free: make as many as you like, download a PNG with no sign-up, and get every other format and print files with a free account. Editable codes come with a plan, a pass, Keep-alive or the 14-day trial.
- Can I change the link after printing?
- Yes, with an editable code. It holds a short link you control, so you can point the printed code somewhere new any time, and the next scan opens the new page. A fixed code keeps the link it was made with.
- Can I use my own domain?
- Yes. A custom domain is included on passes, from Starter, and during the trial. Starter covers one subdomain, for example go.yourbrand.com, and apex domains start on Pro.
- What happens if I stop paying?
- Editable codes work to the end of your paid period, and after a failed payment every code keeps working for 14 days. Turn codes back on within 12 months. Fixed codes keep working as long as their links do.
Which form tool should I link to?
Any tool that gives a web link works: Google Forms, Microsoft Forms, Typeform, Jotform or a form on your own site. Pick the one your team already reads.
How many questions should a feedback form have?
Three or fewer for a scan on the spot. One rating, one open question and one optional contact field get far more finished answers than a long survey.
Should feedback and Google reviews share one code?
Keep them apart. A feedback form is private and helps you improve; a review is public and helps new customers choose. Each gets its own code and label.
Can I see how many people scanned the code?
With an editable code, yes: scans are counted by day, so you can compare places, such as rooms against the reception desk.
Do customers need an app to give feedback?
No. The phone's camera opens the form in the browser, and they answer there without installing anything.
